*Job Overview* We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will provide support, guidance, and advocacy to residents and families facing various social, emotional, and environmental challenges. This role involves participation in an interdisciplinary team and is a member of the facility leadership. The Social Worker reviews and prepares pre-admission assessments for placement into the Personal Care Home while consulting with health care and community agencies. This position coordinates and facilitates both the resident and family councils meetings and provides advocacy for both the residents and families. The role also acts as the Privacy and Complaints Officer. *Responsibilities* * a) To facilitate and coordinate all aspects of admissions for placement at Fred Douglas Lodge. * To make preadmission assessments to identify needs, gather knowledge and to provide information to the applicant and family. * To provide tours of the facility to the applicant and/or family. * Advocate for clients by connecting them with community resources such as housing, healthcare, education, and employment services. * Maintain detailed case records and documentation in compliance with organizational policies. * Collaborate with multidisciplinary teams to coordinate services and ensure holistic support. * Educate clients on available programs and assist with navigating complex systems. * Monitor progress and adjust care plans accordingly to achieve desired outcomes. * Develops a psychosocial history at pre-admission, in collaboration with the resident and family, to ensure that the health care team has the information required to regard the resident as a person deserving of respect and dignity. * Assess the applicant and family in terms of their expectations and acceptance of placement in the personal care home. * Facilitate a smooth transition into the personal care home through interdisciplinary collaboration and development of an admission plan. * Monitors the resident?s adjustment to placement within the personal care home. * Identifies areas of maladjustment, communicates, and advises the interdisciplinary team accordingly. * Ensures that the resident and family are aware of their personal rights and responsibilities, and of the concerns process, including Protection for Persons in Care. * Provides information to the resident and family regarding the process for delegating decision making through the designation of Power of Attorney, Committeeship, Proxy, and Health Care Directives. * Coordinates Care Plan Reviews involving resident and family and participates in resident Care Plan Reviews. * Maintains up to date records of pertinent information regarding the residents, the delivery of social work services, including documentation on resident files. *Requirements* * Bachelor?s degree in Social Work. * Must be registered with the Manitoba College of Social Workers. * Proven experience working with needs of the elderly in a personal care home is highly desirable. * Strong interpersonal skills with the ability to build trust and rapport with clients from various backgrounds. * Excellent communication skills, both written and verbal. * Ability to handle sensitive situations with empathy and professionalism. * Knowledge of community resources, social service programs, and relevant laws/regulations regarding PHIA. * Ability to work independently as well as part of a collaborative team. * Strong organizational skills with attention to detail in documentation and case management.
