*Job Overview* We are seeking a compassionate and dedicated Social Worker or Psychotherapist to join our team. The ideal candidate will work in a medical clinic setting, providing support and resources to patients/clients and their families. This role involves assessing patient/clients needs, developing treatment plans, and coordinating services to ensure optimal patient/client outcomes. The Social Worker/Psychotherapist will play a crucial role in advocating for patients/clients, particularly children, and navigating the complexities of healthcare systems. *Duties* * Conduct comprehensive assessments of patients' social, emotional, and environmental needs. * Develop and implement individualized treatment plans in collaboration with healthcare teams. * Provide case management services to ensure continuity of care for patients. * Facilitate communication between patients, families, and medical staff to promote understanding of treatment options. * Offer counselling and support to patients/clients and their families during difficult times. * Maintain accurate records of patient interactions and interventions in compliance with The Couch Counselling policies. *Skills* * Strong research skills to identify resources and best practices in social work. * Experience in case management. * Ability to provide compassionate patient/client care while maintaining professional boundaries. * Proficiency in assessment methods for evaluating patient progress and program effectiveness. * Excellent communication skills for effective collaboration with interdisciplinary teams. * Empathy and patience when working with children and their families facing challenging situations.
