*Job Overview* Are you looking for a unique opportunity to utilize your knowledge and skills to make a difference in the lives of patients? We are a pharmacy that is highly personalized, and has pharmacists performing home visits. We require a part-time clinical pharmacist. This pharmacist plays an active role in enhancing the qualify of life of our clients. *Responsibilities* * Visit patients in their homes to develop rapport, perform medication reviews, and identify drug-related problems * Assess patients and recommend drug therapy changes to prescribers based on evidence-based literature, knowledge of the patient, and your clinical expertise * Help patients manage their medications to achieve adherence * Counsel patients on their medications and disease states * Assist the patient?s primary care provider and specialists to manage their chronic diseases * Liaise with the patient?s other healthcare partners, as applicable * Perform therapeutic checks of prescriptions to ensure medications dispensed are safe and appropriate * Perform injections in the home * Commit to ongoing continuous education *Skills* * Personable and has excellent communication and interpersonal skills, especially with seniors and those suffering from poor mental health * Strong skills ? and love for ? one on one consultations, problem solving and helping people * Comfortable visiting homes of patients in all areas of the city * Highly motivated and self-directed to apply clinical knowledge and skills * Able to adapt quickly to changing conditions * Willingness to develop relationships with patient?s families and caregivers with the goal of improving patient outcomes * Comfortable in oral and written communication to physicians, nurse practitioners and other professionals * Excellent time-management and prioritization skills *Qualifications* * Licensed pharmacist in good standing * Injection certified * Certified to prescribe for minor ailments, smoking cessation, and UTIs * Must have your own vehicle * Additional certifications from the Board of Pharmacy Specialists is an asset, but not required It is a rewarding position as you are able to see the positive difference you make in a person?s life.
