*Carefirst Family Health Team* is a non-profit, primary healthcare clinic located in Richmond Hill and Scarborough. We offer medical services and healthcare programs to a diverse community in the Greater Toronto Area.
We are currently seeking a *Registered Nurse* to join our clinic in Richmond Hill.
*Terms :* Full Time Permanent
*Location: *Richmond Hill
*Responsibilities:*
* Provide clinical support for patients and physicians, including home visits and community outreach
* Triage patients/clients, conduct health assessments, manage appointment schedules with medical receptionists
* Facilitate patient referrals, provide education and counselling to patients (i.e., chronic disease management and prevention, mental health and addiction, etc.), and conduct patient follow-ups
* Practice within the scope of Registered Nurse (i.e., perform basic nursing skills, administer vaccinations, injections and treatments as prescribed by a physician)
* Ordering and maintaining the clinic?s supplies and equipment
* Manage publicly funded vaccines and inventory (in collaboration with Public Health) including daily cold chain management
* Participate in the development of the MOHLTC program planning, work plan and primary care delivery
* Conduct daily statistics and compile timely reports to ensure continuity of care
*Qualifications:*
* Post-secondary degree in Nursing (BScN) or equivalent
* Current registration as Registered Nurse (RN) with the College of Nurses of Ontario and in good standing
* Current membership with the Registered Nurses Association of Ontario
* 2 years of clinical experience in a primary care setting
* Valid Basic Cardiac Life Support (BLS) certification
* Demonstrated knowledge of nursing theory, techniques principles and practice in order to provide holistic and comprehensive patient care
* Ability to work with both internal and external allied health professionals
* Strong interpersonal skills and verbal and written communication skills, with ability to deliver information and work effectively with patients, families, and an inter-disciplinary team
* Ability to exercise sound clinical judgment and to function in an independent, self-directed manner
* Well organized and ability to multitask in a fast-paced environment
* Demonstrates empathy and sensitivity to patient needs in a culturally diverse practice population
* Proficient computer skills in Microsoft Office and medical EMR software (Telus Practice Solutions)
* Proficiency in English; and a second language reflecting the community served, such as Cantonese and/or Mandarin, is an asset
* Available to work flexible hours including some evenings and weekends
* Current vulnerable sector check (completed within one year)
