*Overview* We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for patients. This position requires a strong understanding of medical terminology, acute care, and patient care practices, particularly in hospital settings and pediatrics. As a Pharmacist, you will collaborate with healthcare professionals to provide optimal medication management and support patient health outcomes. *Responsibilities* * Review and interpret physician orders to ensure accuracy and appropriateness of prescribed medications. * Prepare and dispense medications, including compounding when necessary, while adhering to safety protocols. * Provide medication administration guidance and counseling to patients and healthcare staff. * Monitor patient medication therapy for efficacy, safety, and adherence; make recommendations as needed. * Educate patients on proper medication usage, potential side effects, and interactions with other drugs. * Maintain accurate records of prescriptions and ensure compliance with regulatory requirements. * Participate in clinical rounds in acute care settings to provide pharmaceutical expertise to the healthcare team. * Stay current with advancements in pharmacotherapy and contribute to continuous quality improvement initiatives. *Skills* * Strong knowledge of retail pharmacy operations and medication administration protocols. * Proficiency in medical terminology relevant to pharmacology and patient care practices. * Excellent communication skills for effective interaction with patients, families, and healthcare professionals. * Ability to work collaboratively within a multidisciplinary team focused on enhancing patient health outcomes. * Strong analytical skills for evaluating patient medication regimens and making informed recommendations.
