For more information, visit https://sah.on.ca/join-our-team/attracting-and-retaining-staff/* *PURPOSE:* The ICcare Pharmacy is Sault Area Hospital?s retail pharmacy offering prescription services to the community. The Pharmacist serves patients by preparing medications and teaching patients about their medication; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies. *DUTIES:* 1. Prepare medications for dispensing to patients. 2. Collect and interpret relevant patient information to identify drug related problems as well as determining therapeutic outcomes. 3. Oversee drug distribution, ensuring adherence to policies and procedures. 4. Apply knowledge when dispensing medication. 5. Review prescriptions for accuracy, appropriateness and completeness prior to dispensing. 6. Provide patient medication information and medication teaching as required. 7. Maintain collected patient information. 8. Ensure removal of outdated, mislabeled, deteriorated or recalled drugs for appropriate disposal. 9. Credit and return medications to stock. 10. Manage inventory of prescription and Over-The-Counter (OTC) medications 11. Manage third party payer agreements, reimbursements, and funding sources for medications to ensure financial performance of iCcare Pharmacy 12. Work with Sault Area Hospital staff and programs to support the safe transition to community care 13. Assist with the preparation of the drug budget for the program and with monitoring performance. 14. Document and report any unexpected adverse drug reactions. 15. Function as a professional role model demonstrating good interpersonal skills and a commitment to self-growth and development. 16. Maintain current knowledge of best practices and benchmarks for the delivery of services, appropriate research, literature, accreditation requirements, policy and legislative requirements and implementing improvements/changes as appropriate. 17. Ensure appropriate security and documentation to meet all expectation of Ontario College of Pharmacists, NAPRA, and pertinent provincial or federal laws related to the retail pharmacy operation. 18. Other duties as assigned. *Staffing and Licensing Requirements: * 1. Bachelor?s or Doctoral degree in Pharmacy with formal clinical training 2. Current registration with the Ontario College of Pharmacists in good standing *Knowledge and Skills: * 1. Ability to work effectively and efficiently as a team member in a fast paced environment in stressful situations. 2. Customer service, interpersonal and telephone skills. 3. Computer literacy in a Microsoft environment. 4. Ability to maintain successful working relationships with the interdisciplinary team to achieve positive patient outcomes. 5. Ability to organize time effectively to perform the duties of the position. 6. Ability to meet and interact with people in a pleasant, professional, responsible and reassuring manner. 7. Behaviour consistent with Hospital Mission, Values and Standards of Performance. 8. Human relations skills including empathy, sensitivity, understanding of human and organizational behavior, motivational techniques and counseling skills (e.g. teaching patients). 9.
