Job Description Delivers primary health care to residents in long-term care in the management of acute and chronic medical conditions, therapeutic management, health promotion and disease/injury prevention to deliver comprehensive health services -Provides clinical leadership to LTC team members centered around resident care needs which can include resident admission/discharge, hospital discharge planning, palliative and end-of-life care, pain management, dementia care and responsive behavior management -Conducts health assessment using and adapting assessment tools and techniques based on resident needs and performs a completes health history, including physical, psychosocial, emotional, cultural and ethnic dimensions of health -Performs physical examination and identifies and interprets normal and abnormal findings and orders diagnostic investigations and interprets results using evidence-based clinical reasoning -Collaborates and- LocationPeterborough, ON
