Gloucester Family Health Centre is looking for a *Nurse Practitioner (NP)* to join our busy clinic and support our team of family physicians in delivering top-quality care. This position involves managing a dedicated caseload of patients while working collaboratively with other healthcare professionals to provide comprehensive primary care services.
*Key Responsibilities:*
* Provide direct primary care to a panel of 450-550 patients, including regular assessments, preventative care, chronic disease management, and acute care.
* Work with family physicians to aid with patient consultations, follow-up visits, and care planning.
* Order and interpret diagnostic tests, prescribe medications, develop treatment plans and manage paperwork.
* Collaborate with family physicians and the healthcare team to ensure continuity of care.
* Educate patients on health maintenance, disease prevention, and self-care skills.
* Maintain accurate and timely documentation in electronic medical records (EMR).
*Qualifications:*
* Licensed Nurse Practitioner (NP) with a focus on primary care or family medicine.
* Strong clinical judgment and the ability to work independently.
* Excellent communication skills and a team-oriented mindset.
* Familiarity with EMR systems is a plus.
