*SUMMARY: *
Under the supervision of the nurse-in-charge, the community health nurse (CHN) will support first nations families in reaching optimal health. The CHN will focus on providing holistic care to TFN members by meeting with clients and their families and developing a plan of care with the client. They will achieve this through; assessments and screenings, healthy lifestyle support and education.
*MAJOR RESPONSIBILITIES: *
* Health promotion interventions such as; physical activity, healthy nutrition, substance abuse prevention, STBBI counselling, perinatal health teachings, contraceptive health and harm reduction strategies.
* Participates in program development by collaborating with all health and wellness center workers and local clinics/hospitals.
* Participates in the periodic assessment of the overall health of the community and updates the service delivery plan as needed to best meet the health needs of the members.
* Plan and coordinates; workshops, clinics, and activities with nursing staff and other programs
* Performs all other related duties as assigned by the nurse-in-charge.
*SPECIFIC DUTIES: *
* Organizes and participates in immunization clinics for all age groups.
* Maintain immunization records and electronic medical records of individuals according to provincial standards, protocols and guidelines.
* Capable of carrying out phlebotomy procedure for laboratory clinics.
* Provides information on various health topics according to the Health Canada Calendar
* Acts as liaison between clients and various stakeholders involved: hospitals, Public health Agencies, doctor?s clinics etc.
* Participates in various program investigations such as Tuberculosis (TB), Sexually Transmitted Diseases (STI), and epidemiological investigations, diabetes and cardiovascular prevention and screenings.
* Provide care in the absence of a physician by assessing the client?s condition, establishing a nursing diagnosis and referring to appropriate agency as needed.
* Provide emergency care to clients and ensures surveillance, follow-up and adequate transportation to the nearest hospital.
* Administer therapeutic treatments as directed by the medical professional.
* Quality assurance for all equipment and supplies.
* Participate in meetings with other community workers and educators or stakeholders.
*EDUCATION AND EXPERIENCE REQUIREMENTS: *
* Diploma of nursing or Bachelors of Science Nursing degree.
* Permit from l?Ordre des Infirmiers et Infirmieres du Quebec (OIIQ). Collegue of Nursing of Ontario (CNO) permit will be considered.
* Five (5) years general nursing experience. Acute care or public health experience is an asset.
* Experience in project proposal development and policies and procedure writing skills.
* Basic Life Support (BLS) certification.
* Cultural sensitivity and trauma informed care is an asset.
*KNOWLEDGE AND SKILL REQUIREMENTS: *
* Strong assessment skills.
* Have knowledge of the Timiskaming First Nation (TFN) services, administrative and operational policies and procedures.
* Good communication skills, can work independently and within a team
* Intermediate computer skills.
* Knowledge of First Nation culture is an asset.
* Contribute to the development and maintenance of a co-operative and stimulating work environment.
* Bilingual (English/French).
*WORKING CONDITION: *
* Monday to Thursday 8:00 am ? 4:30 pm
* Friday 8:00 am ? 12:00 pm
* Overtime may be required to meet deadlines and complete projects.
* Reimbursement of OIIQ registration fees and mandatory trainings.
* Remuneration according to the FIQ salary scale.
Job Types: Full-time, Permanent
Pay: From $25.81 per hour
Expected hours: 35 per week
