Work schedule may be changed to meet operational needs.LANGUAGE REQUIREMENTS: Bilingual essentialNOTE: Appliacants must attach a resume to their application.JOB SUMMARY :Under the management of Primary Care, the Metabolic Health Trajectory Coordinator provides clinical leadership and coordination of programs and clinic staff; evaluates the current model of care and processes to develop a new interdisciplinary care model better aligned with the evidence; and implements the new care model to regionalize the program within Vitalité Health Network. MetabolicHealth trajectory includes the MotivAction Youth Clinic and the Community Bariatric Service. The metabolic health trajectory aims to prevent risk factors and promote healthy lifestyle habits. The Motivaction Youth Clinic focuses on the prevention and treatment of obesity in children and young people, and the Community Bariatric Service includes pre- and post-operative follow-up.REQUIREMENTS: Bachelor's degree in a health care discipline from a recognized university; Member in good standing of their association; Minimum of five years' experience in a clinical setting; Minimum of two years' experience in program development or clinical coordination; Proficiency using Microsoft Office tools; Demonstrated leadership, resource management, communication, project implementation and decision-making skills; Initiative, application and professional judgment; Problem-solving skills; Ability to organize and lead a multidisciplinary team; Ability to maintain effective working relationships with clients, families, co-workers, partners and the public; Ability to work independently and as part of a team; Physical ability to perform assigned work; Good work history (performance and attendance); Adherence to professional ethics principles, the Network's philosophy and organizational values; Adherence to the Network's confidentiality rules.NOTE: Position to be reviewed by the Part III Classification Committee.
